Fatal Collision Involving a Pedestrian on South Main Street

Madisonville, KY (Hopkins County) - The Kentucky State Police is investigating a two-vehicle collision involving a pedestrian fatality that occurred on South Main Street in Madisonville, KY, on Sunday, April 13, 2014, at 5:08 p.m.

The preliminary investigation revealed that Karen W. Daniels, 50, of Madisonville, KY, was operating a 2003 Mercury Sable southbound on South Main Street. Amy M. Terry, 37, of Madisonville, KY, was operating a 2001 GMC Safari Van, also southbound on South Main Street. Ms. Daniels stopped in the right travel lane to offer a ride to Sarah A. Fraley, 19, of Madisonville, KY, who was walking on the sidewalk. Ms. Fraley was walking around the vehicle to enter on the back driver's side as Ms. Terry was approaching. Ms. Terry struck the left side of Ms. Daniels' vehicle, also striking Ms. Fraley.

Ms. Fraley was transported via ambulance to Baptist Health Madisonville where she was pronounced deceased by Dennis Mayfield, Hopkins County Coroner. There were no other injuries from the collision.

Trooper Luis Palmer is continuing the investigation. Sergeant Nick Rice, Trooper Jeff Ayres, Trooper Nathan Craft, Madisonville Police Department, and Medical Center Ambulance Service assisted at the scene.
