Kentucky State Police Investigates a Two Vehicle Fatal Collision in Breckinridge County

HARDINSBURG, Ky. (July 27, 2025) – Kentucky State Police (KSP) responded to a two-vehicle collision in Breckinridge County Saturday evening that resulted in three people dead.  

On July 26, around 5:10 P.M. CDT, KSP Post 4 received a call from the Breckinridge County Dispatch requesting KSP investigate a two-vehicle collision near the 13000 block of KY 261.

The preliminary investigation revealed that Joseph Maynard, 48, of Owensboro, was operating a Ford F-150 southbound on KY-261 when, for unknown reasons, he crossed the center line and struck a Chevrolet Silverado head-on, causing it and the Ford to become engulfed in flames. 

The operator of the Silverado, Hunter Lyons, 22, and a one-year-old juvenile passenger was pronounced deceased at the scene by the Breckinridge County Coroner’s Office. An adult female passenger in Lyons’ vehicle was flown to the University of Louisville Hospital with non-life-threatening injuries. Maynard was also pronounced deceased at the scene by the Breckinridge County Coroner’s Office.

The affected portion of KY 261 was closed for several hours to allow troopers to reconstruct the collision. KSP Post 4 was assisted at the scene by the Breckinridge Co. Sheriff’s Office, Breckinridge Co. EMS, Hardinsburg Fire Dept., Fordsville Fire Dept. and the Breckinridge Co. Coroner’s Office. The collision remains under investigation by reconstructionist Trooper John Goldsborough.
